Clasificación TNM para Tumores del Timo
Basado en la 9na Edición del Manual de Clasificación de Tumores Malignos
Reglas de Clasificación
La clasificación se aplica a tumores epiteliales del timo, incluyendo timomas, carcinomas tímicos y tumores neuroendocrinos del timo. No se aplica a sarcomas, linfomas y otros tumores raros. Los cambios en esta edición se basan en recomendaciones del Proyecto de Estadificación de la Asociación Internacional para el Estudio del Cáncer de Pulmón (IASLC). Debe haber confirmación histológica de la enfermedad.
Ganglios Linfáticos Regionales
Los ganglios linfáticos regionales son los ganglios anteriores (peritímicos), los ganglios intratorácicos profundos y los ganglios cervicales.
Clasificación Clínica (cTNM)
T – Tumor Primario
- cTX: El tumor primario no puede ser evaluado.
- cT0: Sin evidencia de tumor primario.
- cT1: Tumor limitado al timo, con o sin encapsulación, o que invade directamente la pleura mediastínica pero no involucra ninguna otra estructura mediastínica.
- cT1a: ≤ 5 cm en su mayor dimensión.
- cT1b: > 5 cm en su mayor dimensión.
- cT2: Tumor con afectación directa del pericardio (parcial o de espesor total), el pulmón o el nervio frénico.
- cT3: Tumor que invade directamente cualquiera de los siguientes: la vena braquiocefálica, la vena cava superior, la pared torácica o las arterias o venas pulmonares extrapericárdicas.
- cT4: Tumor con invasión directa a cualquiera de los siguientes: la aorta (ascendente, arco o descendente), vasos del arco, arteria o venas pulmonares intrapericárdicas, el miocardio, la tráquea o el esófago.
N – Ganglios Linfáticos Regionales
- cNX: No pueden ser evaluados.
- cN0: Sin metástasis ganglionar regional.
- cN1: Metástasis en ganglios linfáticos anteriores (peritímicos).
- cN2: Metástasis en ganglios intratorácicos profundos o cervicales, incluyendo paratraqueales, subcarinales, ventana aortopulmonar, hiliares, yugulares y supraclaviculares.
M – Metástasis a Distancia
- cM0: Sin metástasis.
- cM1: Metástasis a distancia.
- cM1a: Nódulo(s) pleural(es) o pericárdico(s) separados.
- cM1b: Nódulo(s) intraparenquimatoso(s) pulmonar(es) o metástasis a distancia.
Clasificación Patológica (pTNM)
Las categorías pT y pN corresponden a las categorías cT y cN, respectivamente.
